ACT.md and Boston Allied Partners Strengthen LTSS Care Coordination for Massachusetts Medicaid Members
(PRWEB) July 20, 2018 -- ACT.md, a national provider of health collaboration software, today announced a partnership with Boston Medical Center and Boston Allied Partners supporting care for children and adults who are Massachusetts Medicaid (MassHealth) members who need long-term services and supports (LTSS).
Boston Allied Partners is a consortium of health and human services organizations with over 40 years of combined experience providing home-based care services. The consortium is one of eight Community Partner organizations designated by MassHealth to strengthen care coordination for MassHealth members with complex LTSS needs, and to assist members with living healthfully and independently in the community. The consortium is led by Boston Medical Center and joined by the three Aging Services Access Points that serve the Boston area: Boston Senior Home Care, Central Boston Elder Services, and Ethos. They are contracted with 11 newly formed ACOs and MCOs in Massachusetts.
ACT.md provides LTSS programs a HIPAA-compliant, cloud-based health collaboration hub. ACT.md supports cross-sector data exchange, communication, and collaboration between the state’s Medicaid Management Information System and all providers and service agencies. Care coordinators and supervisors at Boston Allied Partners use ACT.md’s data-driven health collaboration hub to:
● capture enrollee information, outreach activities and progress through the program,
● create and share patient-centered, goal-oriented care plans, and
● facilitate communication, collaboration, and coordinated action around LTSS needs.

More than 12 million people in America – primarily older adults and adults and children with intellectual and developmental disabilities, physical disabilities, and mental health conditions – rely on Medicaid-funded LTSS to meet their personal care needs across a variety of community and institutional settings. These services include direct help with routine activities for daily self-care and functioning, (e.g., eating, bathing, and dressing) and with activities that allow an individual to live independently (e.g., housework, meal preparation, and grocery shopping). Medicaid is the single leading payer of these critical services, spending more than $140 billion annually – a sum which will grow 18% by 2020 and double by 2060.**
