ACT.md's CareHub™ Platform Selected for Whole Person Care and other Community Programs in Santa Cruz County, California

SCHIO is one of the oldest and most advanced multi-stakeholder health information exchanges in the country, connecting more than 100 organizations across Santa Cruz County, including physician groups, hospitals, federally qualified health centers, county clinics, providers of mental health and substance use disorder services, and community service providers.
