Alameda County Partners with Elemeno Health to Advance Clinical Best Practices
OAKLAND, Calif. (PRWEB) April 19, 2018 -- Elemeno Health, the developer of a proprietary cloud-based solution for front-line healthcare teams, announced that Alameda County has partnered with Elemeno Health to expand implementation of the Elemeno solution in multiple Federally Qualified Health Clinics (FQHCs) within Alameda County.
The Health Program of Alameda County (HealthPAC) is the first to deploy Elemeno’s solution in FQHCs to engage front-line healthcare teams in the consistent delivery of best practices and improve health outcomes. Specifically, the solution has transformed clinical protocols for colorectal cancer screening and opioid safety into interactive decision guides and reference materials to support providers with key discussion points.

Developed in collaboration with UCSF Medical Center, the Elemeno solution provides “just-in-time” training, enabling front-line medical professionals to quickly access, from any device, interactive guidelines, smart checklists and concise how-to videos, 24/7. Complicated best practices are simplified into bite-sized learning, and the social and “gamification” elements included in the software are designed to engage and motivate team members.

Last November, Alameda County launched a pilot program for colorectal cancer screening at multiple clinic sites of the FQHC Tri-City Health Centers. The pilot received favorable response from patients who feel more engaged with their doctor and providers who appreciate having vital information at their fingertips during the care encounter. In March, the team launched a second protocol for opioid safety and pain management. The pilot is already demonstrating important benefits including greater patient receptivity to discussions about pain contracts and tapering their medication dosage.

Elemeno’s solution is designed to advance quality and patient safety at the point of care. In a one-year study at UCSF Benioff Children’s Hospital San Francisco, the Elemeno prototype was deployed to target central line care. The results, published in the American Journal of Medical Quality, showed that the platform helped UCSF implement best practices and cut central line infections (a costly and deadly hospital-acquired infection) in half, saving over $1 million and an estimated three lives.
