High-performing community health systems and providers can apply now to join this groundbreaking collaboration to improve care quality and increase cost efficiency. This opportunity is open to those new to value-based Medicare payment as well as those with prior experience.
Health providers can be of any size and anywhere in the United States to take advantage of this exciting opportunity. As part of the 90-day application process, providers will be pre-qualified before entering into a contract with Caravan Health and its ACO partners.
In an ACO, community health systems and providers work together to provide high quality care and improve cost efficiency. Caravan Health-affiliated ACOs have been national leaders in quality and reducing total costs for Medicare, saving the program $54 million in 2017 and returning $15 million of that directly to participants. Caravan Health launched the country's largest ACO in 2019, bringing together independent physician groups and hospitals, along with smaller ACOs to gain enough scale to achieve even greater and more stable results.
