As the nation grappled with COVID-19 and new virtual service regulatory changes, Caravan Health accelerated its work with the patient engagement platform to provide its clients with tools to manage and monitor COVID-19-related cases more effectively.

The application enables clinicians to send a daily, comprehensive account of potential COVID-19 symptoms to their Medicare patients. Patients can self-report their temperature, communicate social determinants of health and request a follow up phone call. Clinicians can track their patient’s reports directly through the application more efficiently without having to wade through excel spreadsheets and individual notes. The technology also helps protect patients and providers from unnecessary risk.
