Catholic Health Partners with HRS for a Palliative Care Telemonitoring Pilot Funded by DSRIP
Catholic Health Home and Community Based Care has partnered with Health Recovery Solutions (HRS) for a Palliative Care Telemonitoring Pilot. Since the program's inception in April 2018, there have been no recorded hospital admissions among participants.
BUFFALO, N.Y., Aug. 7, 2018 /PRNewswire-PRWeb/ -- Catholic Health Home and Community Based Care has partnered with Health Recovery Solutions (HRS) for a Palliative Care Telemonitoring Pilot. The pilot is a joint initiative with Catholic Health's community partner, Hospice Buffalo, which was established to reduce avoidable emergency department (ED) visits and hospital readmissions. Patients with chronic diseases who require palliative care interventions are eligible to participate in the pilot.
The Palliative Care Telemonitoring Pilot is in the unique position of having been chosen to receive Delivery System Reform Incentive Payment (DSRIP) funding from New York State for one year. Catholic Health plans to aggregate the data from the pilot program in order to build an evidenced based case supporting insurance reimbursement for telemonitoring and virtual visit engagement. Since the program's inception in April 2018, there have been no recorded hospital admissions among participants.
The patients placed on Catholic Health's palliative care telemonitoring program are provided with 4G tablets loaded with the HRS software. The tablets are paired with Bluetooth devices, which allow patients to take their blood pressure, weight, heart rate, and pulse oximetry readings. The software gives clinicians the ability to monitor patient vitals, respond to risk alerts, and conduct nursing visits through video, phone, and text chat all in real time.
Catholic Health is particularly excited about the ability to conduct video visits with patients. Videoconferencing allows the clinicians at Catholic Health to increase touch points with patients, while simultaneously decreasing clinician travel time. Catholic Health reports that their patients are equally excited about this unique care delivery model.
