VANCOUVER, Wash., Oct. 22, 2019 /PRNewswire-PRWeb/ -- Eden Health has partnered with Health Recovery Solutions (HRS), a nationwide telehealth provider, to bring telehealth and remote patient monitoring services to high-risk patients at ten home health and hospice branches across five states: Washington, Idaho, Nevada, California, and Arizona.
Since August 2019, Eden Health has offered 24/7 patient monitoring and targeted support to COPD and CHF patients with an increased risk of rehospitalization and to patients who would benefit from daily monitoring and clinical support. In addition, telehealth services leveraging video conferencing and medication tracking are available to hospice patients in rural communities, creating an additional touchpoint between clinician and patient.

Eden Health, in tandem with their home health and hospice telehealth programs, has also established a private pay program to extend care to patients outside of home care, assisting in the continued care and disease management.
To engage patients in their care, Eden utilizes the HRS platform, which provides patients with medication reminders, educational videos and quizzes, daily symptom surveys, and seven Bluetooth peripheral devices to record daily vitals. To provide caregivers with peace of mind and ensure patients are receiving care when they need it most, all patient data is uploaded in real-time to the clinician's dashboard. This allows clinicians to monitor patients continuously, communicate with patients, and stage clinical interventions via text messaging, phone, and video conferencing platforms.

Patients have several avenues by which to enroll in Eden Health's telehealth program. Eligible patients are referred by the hospital, skilled nursing facility, or primary care physician upon referral to Eden's Home Health agency. Additionally, Eden's home care team will enroll high-risk patients following clinical evaluation.
Eden's telehealth program is a patient-centered approach aimed at reducing rehospitalizations through improved communication, patient education, and disease self-management. Each patient enrolled in the telehealth program receives a care plan specific to their needs and condition(s), as well as around the clock monitoring and support from an RN-led triage team and visiting home health clinical team.
