Fit4D and Humana to Co-Present at the AADE National Conference
NEW YORK (PRWEB) July 25, 2018 -- Fit4D, a New York City-based diabetes management company, today announced that its CEO David Weingard and Clinical Director Brooke Benton will join Humana Inc.’s (NYSE: HUM) Bold Goal Director Dr. Andrew Renda on the main stage at this year’s annual American Association of Diabetes Educators (AADE) conference to discuss how value can be delivered for patients, providers and payers.
Humana, a leading health and well-being company, and Fit4D will share successful member engagement strategies and lessons learned in the real world through their partnership. The session will take place on Saturday, Aug. 18 at 9.15 a.m. in room 316 of the Baltimore Convention Center. The annual AADE conference brings together health industry experts to provide a global perspective to the discussion of critical issues and new models of diabetes care.
The Humana and Fit4D partnership, which has expanded in Florida over the last year, has been effective with enrollment rates topping 30 percent and delivering a positive impact on patient’s health with diabetes as measured by both blood sugar control and Healthy Days. Healthy Days is a survey tool developed by the Centers for Disease Control and Prevention to track self-reported health-related quality of life. The program is available for both English and Spanish speaking members.
“We are honored to host this session with Dr. Renda, whose passion for improving the health of Humana members is inspiring,” said David Weingard, Fit4D CEO. “The national AADE conference is the perfect forum to share successful engagement strategies as it celebrates the expertise, passion and commitment of diabetes educators across the U.S. We will share how measurable health improvements have delivered in value-based care models to achieve outcomes, improve quality measures, and drive patient empowerment.”
“It’s important that we highlight this example of how certified diabetes educators can help address social determinants of health as part of a comprehensive care model to improve diabetes outcomes,” said Dr. Renda. “Humana is committed to improving health and quality of life through our Bold Goal population health strategy, where we aim to help the communities we serve become 20 percent healthier by the year 2020.” By partnering with Fit4D, we are testing a high tech, high touch diabetes care model that includes addressing root causes of uncontrolled diabetes.”
