PHILADELPHIA, March 26, 2019 GSI Health, a leading population health management technology company, today announced that its GSIHealthCoordinator platform has been implemented as the care management technology of choice for New York's Care Compass Network (CCN). CCN is a nonprofit community organization that champions new models of care to the community with a focus on delivering quality care while reducing inefficiencies through enhanced care coordination and community-based engagement and education. CCN collaborates with over 200 partner organizations, including hospital systems and a range of community-based organizations.
CCN's partnership with GSI Health enables disparate providers to collaborate on those patients they treat together and develop a joint care plan that includes social determinants of health to address all patient issues. The network uses GSIHealthCoordinator to integrate key systems such as electronic health record (EHR) and care management systems and regional health information organizations (RHIOs) to facilitate fast, efficient care management supporting the Medicaid population in New York's Southern Tier. A multi-instance implementation with organization-specific, autonomous workflows and information sharing across the community ensures that all stakeholders can access the information they need to improve patient outcomes.
"We are excited to launch GSIHealthCoordinator and provide critical community agencies with a significant technological boost. Bringing this technology to the Southern Tier will enable our network to further provide quality care to our communities by enabling the alignment of care plans," said Mark Ropiecki, Executive Director of Care Compass Network. "We look forward to further rolling out this innovative platform across our network."
"We have deep experience enabling collaboration across diverse community-based organizations to support whole-person care, capture and address social determinants of health, and improve patient outcomes," said LeRoy E. Jones, Founder and CEO of GSI Health. "We look forward to a long-term partnership in which our technology empowers CCN stakeholders to manage their care management goals as value-based care models evolve into the future."
GSI Health's GSIHealthCoordinator platform brings diverse organizations together to collaborate as care teams with harmonized workflows so that care delivery across the patient population is comprehensive and efficient. GSIHealthCoordinator combines care management, analytics, and a robust interoperability layer to enable organizations to figure out who to focus on and what to do, and analyze how to improve care management strategies to maximize impact. GSI Health currently supports more Health Home programs in New York than any other solution and four of the largest DSRIP/1115 Medicaid Waiver Performing Provider Systems in the state, providing deep experience in the state's regulatory environment.
