The article, authored by Dr. Brian K. Iriye and Dr. Manijeh K. Kamyar, reports outcomes from more than 3,500 annual deliveries across Hera's six hospitals and seven offices during 2024–2025.
Under the model, MFM subspecialists design and govern evidence-based protocols that are diffused across OB/GYNs, advanced practice providers, certified nurse-midwives, obstetric hospitalists, and embedded behavioral health and addiction medicine providers. Rather than functioning solely as consultants for high-risk referrals, MFM subspecialists serve as the clinical architects of standardized care pathways spanning preterm birth prevention, low-risk cesarean reduction, diabetes management, maternal opioid use disorder, and preeclampsia prevention.
Across these five domains, Hera's outcomes compared favorably with county, state, and national benchmarks. The preterm birth rate was 10.0%, compared with 11.4% in Clark County. The nulliparous, term, singleton, vertex cesarean rate was 24.6%, compared with 28.5% in Clark County. Self-monitored blood glucose adherence among patients with diabetes in pregnancy reached 96%, compared with a U.S. average of 68%, and low-dose aspirin use among eligible patients for preeclampsia prevention reached 95%, compared with fewer than 50% at baseline. Applying published per-case costs, the model was associated with an estimated $4.03 million in annual savings from preterm birth prevention and low-risk cesarean reduction alone.

The article situates the Hera model alongside existing maternity care frameworks, including Centering Pregnancy, the California Maternal Quality Care Collaborative, the CMS Strong Start initiative, and midwife-led care models, and notes that it is the first community-based system to report outcomes in which MFM subspecialists serve as the organizing clinical authority across the full scope of obstetric care delivery, in both the inpatient and outpatient setting. The authors describe the model as directly operationalizing the Society for Maternal-Fetal Medicine's 2025 special statement calling for a transformed maternity care team.

They call for future prospective studies with concurrent control groups and multi-site replication to validate the findings.
