PALM BEACH GARDENS, Fla., Jan. 13, 2021 /PRNewswire-PRWeb/ -- Longevity Health Plan, the nation's fastest-growing independent provider of Medicare Advantage ISNP plans for nursing home residents, today announced it has expanded into North Carolina. The company is licensed to offer its unique ISNP plan in 30 counties across the state.
"We are pleased to partner with leading skilled nursing providers to bring Longevity's clinically driven, patient-centric model to North Carolina," said Dr. René Lerer, CEO of Longevity. "Caring for the medical, social and emotional needs of individuals living in nursing facilities requires a comprehensive, coordinated and proactive approach. Longevity nurse practitioners, who care for members on-site as an integral part of a nursing home's team, are the keystone of Longevity's multidisciplinary, holistic approach, which also engages pharmacists, geriatricians, behavioral health experts and other clinicians."
"Particularly in these challenging times, our nurse practitioners are a welcome addition to the facility's team, coordinating with primary care doctors and working closely with nursing home staff to ensure high-quality care and more personal attention for members, and peace of mind for their loved ones," Lerer added.
Longevity's health plans, known as Institutional Special Needs Plans (ISNPs), are specially designated Medicare Advantage plans designed for people living in nursing homes. The company creates partnerships with skilled nursing facilities to offer its plan to their long-term residents. Leveraging its nurse practitioners, telehealth, other onsite capabilities and advisory services, the company's model of care focuses on treating in place instead of unnecessarily transferring members away from the security of the skilled nursing facility to the hospital.
"Longevity's value-based partnership model is attractive to nursing home operators and administrators because it delivers high satisfaction for residents and their families, provides additional resources dedicated to patient care, and supports the achievement of quality objectives such as reducing avoidable hospitalizations," said Marc Hudak, chief growth officer for Longevity. "Our growth to date is a testament to the value we bring to skilled nursing facilities especially in the current environment."
Individuals who have Medicare Parts A and B and reside in or are expected to reside in a participating nursing facility for 90 days or more are eligible to join Longevity Health Plan. Longevity's North Carolina plan is currently collaborating with 60 nursing homes in the state.
Longevity's plans provide all the benefits of Original Medicare plus a dedicated on-site clinical team, Part D prescription drug coverage, dental, vision, podiatry, hearing, telehealth, tai-chi and chair yoga, and companion services, among other benefits tailored for North Carolinians.
