Dr. Rajan will co-present the session "Caring for the Complex Medically Ill" along with Mauricio Sardan, MD, a clinician from Sound Physicians, a provider-founded and led organization whose goal is the optimization of post-acute care.
In their session, Dr. Rajan and Dr. Sardan will define what constitutes a "medically complex" patient, in addition to variants of this population, the chronically critically ill. An additional learning objective for attendees will include analysis of models within anesthesia, pediatrics, and how MCG care guidelines define "multiple condition management." Finally, the presenters will cover what clinicians and/or hospital systems have been doing to address the needs of this population in relation to scheduling/staffing, readmission risk reduction, and post-acute care planning.
