Humana Medicare Advantage members in the Philadelphia five-county region now have in-network access to Patina's unique primary care, delivered through virtual and in-home visits

(NYSE: HUM), have signed an agreement that expands the primary care options for Humana Medicare Advantage members in the Philadelphia five-county area.
As of July 1, 2023, Patina became an in-network primary care provider for individuals in Philadelphia, Bucks, Montgomery, Chester and Delaware counties enrolled in Humana Medicare Advantage plans.
Patina supports patients through a dedicated team that personalizes the care experience, including a Patina primary care practitioner (PCP) and a Patina health champion, who helps with everything from scheduling appointments to navigating the healthcare journey and coordinating community resources. By delivering primary care through virtual video visits and in-home visits when needed, Patina helps its patients avoid the time and hassle of driving, parking and waiting for appointments in a physical office.

When eyes-on, hands-on care is needed, Patina clinicians visit patients right in their homes, connecting to other members of the patient's care team virtually via remote monitoring tools to share patient vitals and other health-related information.
This news expands on Patina's market momentum and enables access to Patina for nearly 300,000 Medicare Advantage members in the Philadelphia area, including most major Medicare Advantage plans in the region. Members who select Patina as their primary care provider get in-network access to Patina's dedicated care team.
