Physicians East is the newest partner to choose PharmaPoint's XchangePoint platform to promote improved patient outcomes through pharmacy interventions.
BIRMINGHAM, Ala., July 12, 2022 /PRNewswire-PRWeb/ -- Physicians East is the newest partner to choose PharmaPoint's XchangePoint platform to promote improved patient outcomes through pharmacy interventions. In addition, PharmaPoint will manage the accreditation process for Physicians East's in-house specialty pharmacy.
XchangePoint will be used to monitor Physicians East's Accountable Care Organization (ACO) population to ensure optimal adherence to medication treatment plans for high-risk patients.

A phased rollout will encompass chronic diseases such as diabetes, hypertension and congestive heart failure. ACO patients with these diagnoses will have their medication fill histories reviewed and monitored to ensure there are no gaps in therapy resulting in nonadherence to a prescribed treatment plan and possible adverse events.
PharmaPoint has managed Physicians East's retail pharmacy, Pharmacy East, since 2016 and will now guide facility leadership through the accreditation process for specialty pharmacy from the Accreditation Commission for Health Care (ACHC).
