Repisodic Choice makes it easy to create, share, and manage lists of post-acute care options, engaging patients in the selection process and streamlining the discharge workflow while ensuring compliance with Medicare Discharge Planning Rules. The integration available in App Orchard makes it easier for health systems to adopt their technology and integrate it with existing care management workflows.

Repisodic Choice can be launched directly from Epic by hospital users, who can then search Repisodic’s national, proprietary dataset of post-acute providers, match those providers to each patient’s unique needs, and then distribute those options to patients and their families.
Since their latest funding round in 2019, Repisodic has expanded nationally and enhanced its product capabilities to include digital form signing, as well as a new data analytics suite for health system executives.
