Senior Independence Chooses PREMEDEX for Hospital to Home Program
Toledo, Ohio (PRWEB) December 18, 2013 -- Senior Independence has chosen the PREMEDEX platform as the technology that helps them support seniors in the Toledo area.
Senior Independence and Northwest Ohio ACO are partnering to coordinate care and improve quality and patient satisfaction through their program called Home to Stay. The program, provided at no cost to eligible NW Ohio ACO patients, helps patients with complex care needs make successful transitions from hospital to home.
PREMEDEX enables Home to Stay teams to provide high-quality services and assist with identifying and reducing duplicative medical services and unnecessary hospital readmissions. Plus, PREMEDEX systems include the latest security technologies and meet all HIPAA requirements.
The PREMEDEX system enables Senior Independence and NW Ohio ACO to identify eligible Medicare patients discharged from Toledo-area hospitals who would benefit from a care-transition program.
Home to Stay teams follow up with patients five times after their return home to coordinate home care. Team members educate patients, coaching them on managing their chronic conditions and medications to ensure they remain on the path to wellness.
Case managers and nurses use the PREMEDEX platform, which is available on tablet computers, to perform assessments for falls and medication management during in-home visits and follow-up phone calls.
PREMEDEX allows Home to Stay teams to report key metrics back to NW Ohio ACO as part of the patient's care. The software also is used to share patient information with the referring physician so transitions of medical care are monitored and managed and health concerns do not become issues that cause re-hospitalization.
