PHILADELPHIA and EAST WINDSOR, N.J., May 29, 2019 /PRNewswire-PRWeb/ -- SpectraMedix, a leader in healthcare payer and provider business intelligence, and GSI Health, a pioneer in care management for population health, today announced an integration that brings information from health systems, behavioral health providers, and community-based organizations together with next-generation analytics and actionable insight-driven care management at Care Compass Network (CCN). CCN is a nonprofit community organization that delivers quality care while reducing inefficiencies through enhanced care coordination and community-based engagement across New York state's Southern Tier.

The integration of the SpectraMedix and GSI Health platforms enables CCN to realize its population health strategy by utilizing a broad range of data sources from across CCN's stakeholders. The integration uses these data sources to identify at-risk populations and distribute those patient segments to the appropriate care settings where gaps in care and measures can be closed, and the social determinants of health negatively impacting patient health can be identified and mitigated. Together, the SpectraMedix and GSI Health platforms put the population health analysis into population health action across CCN's network of community-based organizations. This integration creates better understanding of populations, processes, and results that will improve how care is contracted for and ultimately delivered in the southern tier of New York State.

The SpectraMedix Platform™ aggregates data via APIs, FHIR and other interfaces across EMRs, behavioral health, RHIOs, Department of Health and CCN partners, as well as key demographic and SDOH data from the GSIHealthCoordinator care management platform. This data is run through an advanced ETL process within a Master Data Management environment and creates an Enterprise Master Patient Index. The solution then generates population health analytics, identifies measure and care gaps, and creates risk profiles. GSIHealthCoordinator then drives these actionable insights into interdisciplinary workflows across CCN's network of providers, care management agencies, and community-based organizations so care teams can collaborate to improve care delivery, mitigate social determinants negatively affecting patients, and ultimately improve patient outcomes.
