In furtherance of its stated mission of providing exceptional pulmonary care, Sunrise Medical Management chooses HGE Health’s digital symptom monitoring platform for remote patient monitoring in Arizona

"COPD patients are at risk for severe outcomes of COVID-19 infections, receiving daily symptom reports via HGE Health is expected to help us quickly react to possible infections as COVID-19 infections are expanding here in Arizona, " explained Christina Pendleton, Sunrise’s practice manager.

Sunrise Medical Management LLC (“Sunrise”) has selected HGE Health’s remote symptom management platform, HGE Care™, to offer remote patient monitoring services to patients with Chronic Obstructive Pulmonary Disease (COPD) throughout its Arizona network. Patients enrolled in the HGE Care™ platform submit their respiratory symptoms on a daily basis using a mobile application. Patients who report worsening symptom trends are identified for nurse triage and trained registered nurses at HGE Health’s state-of-the-art call center coordinate same-day, remote care for those patients under the supervision of Sunrise’s physicians. HGE’s solution enables scalable, same-day COPD care 365 days a year, including holidays, weekends, and after-hours. Sunrise’s pulmonary team will continue to see the patients for ongoing management of their COPD.

The HGE Care™ platform is available to healthcare providers such as Sunrise as a turnkey solution. Technical resources hosting the platform are managed and maintained by HGE Health, which also provides a staff of nurses to coordinate same-day care for patients, by their primary care or pulmonary physicians to enable timely intervention to address acute exacerbations of COPD symptoms. HGE Care™ data and care coordination encounters are charted in Sunrise’s existing electronic medical record (“EMR”) system, allowing complete care and management of patients without imposing additional demands on Sunrise’s staff.

“Contracting with HGE Health to offer our COPD patients access to the HGE Care™ platform affords us with the ability to maintain an ongoing dialog with those patients between regularly-scheduled or urgent-care visits,” explained Christina Pendleton, Sunrise’s practice manager. “Because such patients are also among the most at-risk for severe outcomes of COVID-19 infections, receiving daily symptom reports is expected to help us quickly react to possible infections while the outbreak of COVID-19 infections is expanding here in Arizona.”

Michael J. Markus, CEO of HGE Health, views Sunrise as an ideal partner for enrolling patients in HGE Care™. “Sunrise has routinely demonstrated a commitment to offering COPD patients access to cutting-edge pulmonary care,” Markus explained. “HGE Health shares a similar desire to facilitate the finest care possible, with unparalleled access to early identification of worsening symptoms and facilitation of timely clinical intervention under the supervision of the patient’s existing provider. Our clinical staff act as an extension of the provider’s office, coordinating care for those in need, allowing Sunrise to oversee the care of COPD patients remotely, on a timely basis, and without significantly adding to the demands on their existing staff.”

The latest deployment of HGE Care™ in Arizona continues HGE Health’s rapid expansion of respiratory services across the United States from its original deployment in Philadelphia. Other markets with a growing presence of HGE Care™ include Pennsylvania, Oklahoma, Texas, Michigan, New Jersey, and Florida.
