Health care teams may soon have to make difficult decisions in the light of Covid-19. There may not be enough hospital beds and ventilators to meet the impending need.

The Your CarePlan video tool can help free needed capacity by recording and honoring the wishes of older patients. When asked, many older adults would choose far less aggressive care than is usually chosen for them. For example, once they learn how unlikely they are to survive CPR and artificial breathing, most older people will choose to forego these painful and ineffective procedures.

Intended for older people or people living in a nursing home, assisted living or senior housing facility, or a decision maker for such a person, Your CarePlan is now available for no cost. The user’s preferences will be printed on a New Jersey POLST form, and when signed by a physician, PA, or advanced practice nurse, becomes a medical order that must be obeyed by physicians and EMT’s.
