2015 SXSW Recognizes Innovator in Chronic Care Management
Austin, Texas (PRWEB) February 12, 2015 -- The 2015 South by Southwest (SXSW) Interactive Festival names CareCycle Navigator® as a finalist for the 18th Annual SXSW Innovation Awards in the Health Med & BioTech Category.
CareCycle Navigator predicts which chronically ill patients in a population are at the greatest risk for impending health problems and gives remote clinicians a structured way to intervene and prevent costly health events. The software, developed by Dallas-based CareCycle Management, makes “connected patient” management a scalable reality and functions as an early warning system for chronic illness exacerbations.
Daily health biometrics, trended data, medical history, social factors, on-going health assessments and current symptom data run through patent-pending algorithms in CareCycle Navigator and a patient-specific dynamic risk score is calculated every time new information comes into the system. Because CareCycle Navigator defines in real-time precisely which patient is most at risk at any moment, clinicians can operate at the top of their license and manage large populations. The application also prompts and tracks disease education and new clinical assessment data such as fall risk, sleep, anxiety and depression.

CareCycle Navigator was selected out of hundreds of applications by a panel of SXSW judges comprised of industry experts and recognized for improving the quality, efficacy, and/or cost-effectiveness of diagnosis, treatment and care in the field of medicine. Winners will be named March 17, 2015. The BHW Group out of Austin, TX provided the web application and technical services for the development of CareCycle Navigator.
