The webinar titled, "Managing Direct Oral Anticoagulant Inhibitor to Heparin Conversion; Heparin Anti-Xa assay or aPTT?", will cover a recently published comprehensive practice guideline on how to manage patient transitions from a direct oral anticoagulant to a heparin infusion, along with an assessment of its performance versus an alternative strategy.
Ever since the introduction of direct oral anticoagulants (DOACs), such as apixaban and rivaroxaban, laboratory staff and clinicians have faced many challenges in determining ideal practices and guidelines when transitions to heparin infusions are needed for patients at high risk of thrombosis. Although heparin infusions are commonly needed for patients on DOACs who are admitted to the acute care setting, little guidance has been provided on how to manage such transitions. To overcome these challenges, clinicians and their laboratory partners have been developing optimal processes to manage transitions to heparin infusions. During Dr. Gulseth's presentation, he will compare heparin anti-Xa assay levels to aPTT to manage heparin infusions and evaluate recent literature developments on how to manage DOAC to heparin transitions. Additionally, he will discuss how to appropriately interpret laboratory values when presented with a patient on a DOAC transitioning to heparin to appropriately manage the transition.
