Quality Care Champions Gather for the 4th Annual Patient-Centered Medical Home Congress

The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. Research indicates the PCMH model improves care quality, the patient experience, and staff satisfaction while reducing health care costs. NCQA’s Patient-Centered Medical Home Recognition Program is the most widely adopted Patient-Centered Medical Home evaluation program in the country. More than 12,000 practices (with more than 60,000 clinicians) are recognized by NCQA.
PCMH Congress is the annual educational gathering for recognized practices, those who are considering recognition, and others interested in patient-centered care. The 3-day meeting attracts nearly 1,000 attendees who will receive tips, tools, and ideas for immediate application through interactive conference sessions tailored to their organization’s needs; engage one-on-one with leading PCMH experts who can assist in answering critical questions; and network with peers from around the country who can share insight into common challenges and offer potential solutions.

An innovator and keen observer of highly successful leaders and teams, Bell will provide an “under-the-hood” look at the characteristics of high-performing teams and explore how to build and nurture teams that consistently perform better, innovate more, and achieve collaborative success.

Also featured on the 2018 agenda is Paul Grundy, MD, MPH, FACOEM, FACPM, the “Godfather” of the Patient-Centered Medical Home revolution. Dr. Grundy is a recipient of the 2016 Barbara Starfield Primary Care Leadership Award and the 2012 NCQA Quality Award. Dr. Grundy will share valuable insights on the changing healthcare landscape and introduce some of the organizations who are driving this change toward a more patient-centered approach.
