StoneGate Senior Living Launches New Central Intake Center to Serve Colorado's Senior Care and Medical Communities
LONGMONT, Colo. (PRWEB) September 19, 2018 -- The new Central Intake Center for StoneGate’s Accel at Longmont and Accel at Golden Ridge will streamline coordinated patient care between hospitals and post-acute care communities
Accel at Longmont and Accel at Golden Ridge, two luxury skilled nursing and rehabilitative communities serving Colorado’s senior population, are announcing the launch of the Central Intake Center (CIC), a free, centralized, referral resource that can be accessed by a simple phone call or fax & is designed to help area medical professionals and administrative personnel identify the best available resources for seniors in need of post-acute medical care.
Accel at Longmont and Accel at Golden Ridge, two facilities that are part of the StoneGate Senior Living family of senior care and housing solutions, have launched the CIC to answer the industry need to provide more cost-effective and time-effective services to patients.
“The centralized referral system assists Colorado’s acute-care providers in delivering fast, appropriate patient admission and bed placement at a post-acute care facility,” said Paula Little, vice president, admissions and analytics at StoneGate Senior Living. “Its mission is to streamline the admission, discharge and referral process for the post-acute care community, delivering excellent patient service in the process.”
The CIC team will provide the region’s caregivers with a centralized, streamlined assessment of patients, ensuring they are placed in the best possible environment. CIC team members will confirm the plan of care, gather clinical and payer information, secure proper payer authorization and place the patient in the post-acute care setting most appropriate for the patient’s needs.
The CIC will make use of a real-time “bed board” program, which monitors the availability of beds and resources for patients’ needs as a resource for the senior care medical community. This allows the CIC team to help transition patients promptly from the hospital to a facility with appropriate resources for their care.
