ACT.md, LifeBridge Partner to Transform Palliative Care
BOSTON, Mass., and SALT LAKE CITY, Utah (PRWEB) March 10, 2016 -- ACT.md and LifeBridge Sherri Project (LifeBridge), a palliative care company, have partnered to provide comprehensive and coordinated services to those diagnosed with terminal illness in an effort to make end-of-life care seamless, compassionate and navigable for patients, their families and caregivers.
LifeBridge is a newly formed organization that has developed a proprietary model and methodology to assure early and appropriate communication, drive timely and relevant care, and support people in the ways that matter to them at the end of their life. Palliative care introduces an added level of complexity that requires thoughtful and strong coordination, reliable processes, and real-time collaboration amongst everyone supporting these patients. This complexity influences utilization, with more than 40 percent of patients seeing 10 or more doctors in the last six months of their lives (Reference 1) and approximately half of total Medicare spending for people who die in a given year allocated to hospital inpatient expenses. (Ref 2) This has been the case for years despite the fact that only 30% of people have discussed their wishes for end-of-life care – a task that 90% of people think is important to do. (Ref 3)
