Alta Home Care LVN Case Manager

During each visit the nurse speaks with the client to assess their current care needs and notes any progress or decline their cognitive or physical abilities. Case Study
A female client, age 90 was referred to Alta on July 11, 2007 from Newport Nursing and Rehabilitation Center in Newport Beach, CA. The original care plan was focused on companionship, basic ADL’s, and common household tasks.
LVN Case Notes:
“My initial evaluation was on January 8, 2014. During my evaluation, I immediately noticed bruising and swelling on both left and right knees and a large bruise on her upper right arm. I was concerned and asked the client what had happened and she told me she had a fall and could not get back up. Fortunately, her caregiver was there and was able to help her back up. The client began to tell me that was not her first fall, which concerned me. Client had weakness in legs and was not stable on her feet. I then recommended PT for stability and balance and the client agreed that would be great. I contacted the area manager and he set the client up with a visit from home health.
My next visit was February 5, 2014 and the client was in great spirits. Since my initial visit, due to the client’s fall, her hours were changed from hourly (5 hours daily) to live in 24 hour care, 7 days a week. I first checked the client’s knees and arm to make sure the swelling and bruising had healed and everything was healed. I assessed her from head to toe to see if she had any other new wounds and found nothing. Her skin integrity was intact and good. She had no other falls since my last visit and she was in great spirits. She said she would like me to visit again.”
Conclusion
