Barnes-Jewish Hospital Rolls Out Epharmix, Innovative Patient Monitoring Program

The participating program, the hospital’s Stay Healthy Outpatient Program or SHOP, helps at risk patients avoid repeated hospital visits and ensures participating patients have access to the care they need at home.
