BluePrint Healthcare IT Selected to Co-Present Three Education Sessions at HIMSS15 with Innovators in Children’s Healthcare, ACO and Regional Care Coordination
Cranbury, NJ (PRWEB) December 18, 2014 -- HIMSS15 Annual Conference and Exhibition has selected three education sessions that will feature BluePrint Healthcare IT clients and subject matter experts in the April 2015 Chicago conference. The sessions will cover children’s healthcare, accountable care organizations (ACOs) and regional care coordination. The three educations sessions are titled:

Under the category of Transitions of Care: Coordination and Management, Dr. Christopher Haines, DO, FAAP, FACEP, Chief Medical Officer of Children’s Specialized Hospital of New Jersey will focus on the valuable lessons that can be learned from Children’s Specialized Hospital’s care coordination program. This model demonstrates how to deliver better outcomes for populations affected by complex healthcare issues through a highly underutilized “lever”: families and caregivers. Children’s Specialized Hospital works with their Family Faculty, a group of parents and caregivers, who provide valuable input to the care model. Colleen Woods, MPA, MPM, Senior Advisor for Healthcare Transformation and Policy for BluePrint Healthcare IT, will lay out the story of how this applies to organizations, whether they are focused on children with complex and costly needs, adults or both.
Under the category of Provider and Patient Engagement, Dr. James Walton, DO, MBA, President and CEO of Genesis Physicians Group and Genesis Accountable Physicians Network (GAPN) will show how providers struggle and overcome the challenge to navigate their new level of responsibility through coordinated care that goes beyond the episodic and involves the entire care team. Sandra Selzer, MSHQ, Vice President, Product Development for BluePrint Healthcare IT, will ensure the session highlights two perspectives, patient and physician, from a newly formed ACO on the road to delivering more efficient and effective care.
Under the Health Information Exchange (HIE) category, Carla Gibson, Senior Program Officer of the REACH Foundation, will show how ten safety net organizations and two Health Information Organizations (HIOs) in the Kansas City region came together to provide access to patient records across the region as well as design, develop and implement electronic transitions of care to improve care coordination among the clinics. Rebecca Goldberg, Principal Consultant, Care Optimization for BluePrint Healthcare IT will demonstrate how the single use case approach, leveraging the Safety Net’s EMR and DIRECT Messaging provided a structured, focused framework, along with a 360 degree readiness assessment.
