Cancer & Hematology Centers Of Western Michigan Implements Navigating Cancer’s Patient Relationship Management Platform

With the cost of cancer care rising and value-based payment systems becoming a reality, many cancer practices are identifying ways to better coordinate patient care while also controlling and reducing costs. For CHCWM, replacing manual, labor intensive systems and redesigning its triage process emerged as two important initiatives in order to remain viable. To do that, they are partnering with Navigating Cancer to put the patient at the center of their care and connect the care team.
The implementation of Navigating Cancer’s Patient Relationship Management solution allows CHCWM to digitize formerly manual, paper-based tasks like depression screening, patient education and tracking patient calls. The practice will also be able to monitor patients when they are away from the clinic so the care team can provide proactive intervention when needed. “Our practice receives a large number of patient calls - many of which are symptom related. We are confident that this software, which includes symptom management pathways, will enable triage nurses to monitor and resolve patient-reported issues in real time, helping us reduce unnecessary emergency room visitations and hospitalizations,” said Dennis Zoet.
Navigating Cancer’s software features a workboard that can be viewed by the entire care team and it stratifies patient needs according to severity - whether it’s a side effect, a medication adherence issue, elevated distress or something else that could negatively impact treatment - and generates alerts as needed. That functionality combined with the remote monitoring program, Health Tracker, and patient engagement tools like the automated and timely delivered of diagnosis and treatment specific information positions CHCWM for success in a changing healthcare landscape.
