Caregiver Homes Serves First Structured Family Caregiving Consumer in Indiana
Indianapolis, IN (PRWEB) November 18, 2013 -- Caregiver Homes, which delivers training, support and financial assistance to family caregivers, is now officially operating in Indiana by serving its first consumer in the state. The company's home-based, long-term care model, Structured Family Caregiving (SFC), recently became available to Medicaid eligible elders and individuals with disabilities who choose to live at home with a caregiver who can meet their care needs while receiving monetary aid and professional supervision from Caregiver Homes.
Caregiver Homes serves over 2,000 individual consumers in four states who have adopted this unique model of care since 2007. Earlier this year, Caregiver Homes of Indiana was approved in the Community Integration and Habilitation Waiver under the DDRS, as well as to recipients of Indiana's Aged & Disabled 1915(c) Home and Community-Based Services Waiver. Caregiver Homes is currently marketing SFC throughout the state. It is opening physical locations within communities where its care teams – registered nurses and professional care managers who leverage a web based care management tool to interact with caregivers – can best serve people and raise awareness of this new care option among families and the network of other professionals who serve and care for them.

To begin serving its first consumer and family caregiver in Indiana, Caregiver Homes worked with CICOA, the Area Agency on Aging and ADRC responsible for that consumer's services and thousands like her in Indianapolis and beyond.
