CareSource Provides Diabetic Food Boxes to 8,000 High-Risk Ohioans
Dayton, Ohio (PRWEB) October 30, 2013 -- CareSource, a nonprofit Ohio-based health plan is launching a statewide campaign to educate its members about healthy eating and controlling diabetes. Research indicates that lifestyle changes of diet are extremely important for people who have diabetic indicators. To help members stay on track and make informed choices, CareSource will provide Diabetic Food Boxes four times a year to approximately 800 members in every Ohio County.
Patient Navigators will provide home and point of service delivery of the Diabetic Food Boxes. The goal is to educate and build relationships with members who have been diagnosed and are high-risk.
The CareSource Diabetic Food Box program is made possible through a $140,000 two-year grant from the CareSource Foundation to The Dayton Foodbank Inc. The Foodbank obtains the food and distributes the boxes to the CareSource Patient Navigators.
“CareSource believes strongly in helping members identify preventative methods for managing a healthier lifestyle. Making a positive impact on the health outcomes of high-risk members throughout the state is an extension of our mission to help the underserved,” Cathy Ponitz, Executive Director CareSource Foundation, Community Relations & Diversity, said.
The Diabetic Food Boxes contain 100 percent non-perishable items.
The contents include:
•Creamy peanut butter
•Brown rice
•Whole wheat spaghetti noodles
•Vegetables (canned low-sodium carrots, tomatoes and mixed vegetables)
•Fruits (unsweetened applesauce and mandarin oranges)
•Dry pinto beans
•Cornflakes
•Chunk chicken
CareSource continues to implement successful member health models, while remaining mission focused to make a difference in the lives of the underserved. The Diabetic Food Box distribution project is currently underway in Dayton, Columbus, Cleveland and Cincinnati.
