Central Virginia Coalition of Healthcare Providers Selects Smartlink Mobile Systems to Address MACRA, Chronic Care Management
CARY, NC, and HAMPTON, VA (PRWEB) September 29, 2016 -- The Central Virginia Coalition of Healthcare Providers (CVCHiP), an Accountable Care Organization providing services in Virginia and North Carolina, has selected Smartlink Mobile Systems as their Chronic Care Management (CCM) solution partner.
With 93% of Medicare dollars spent on beneficiaries who have multiple chronic illnesses, providing more proactive care management services to this population has become critical to success under payment reform. MACRA is a bipartisan legislation that changes how Medicare pays those who provide care to Medicare beneficiaries. The Medicare CCM program is intended to help providers make the transition to value-based care by paying providers to provide monthly, non-face-to-face services to beneficiaries with two or more chronic illnesses. In addition to incentivizing better care management, the CCM program helps to subsidize the necessary investments to succeed under MACRA.
