Children's Specialized Hospital Recognized Nationally as First New Jersey Pediatric Practice Designated a Patient-Centered Medical Home
(PRWEB) July 27, 2015 -- Pediatric Primary Care at Children's Specialized Hospital in Mountainside, New Jersey has become the first pediatric practice in New Jersey to receive a Patient-Centered Medical Home (PCMH) designation by the National Committee for Quality Assurance (NCQA), the most widely adopted model for recognizing PCMH standards.
Through a rigorous assessment process based on scientific evidence and nationally recognized standards of care, Pediatric Primary Care at Children's Specialized Hospital demonstrated their overall dedication to team-based care coordination, population health management, family-centered care and continuous quality improvement in support of all the children they treat who have special healthcare needs.
