Dallas ACO Deploys Innovative Care Coordination Mobile and Desktop App

Developed by BluePrint Healthcare IT, the Care Navigator™ platform offers three components, two of which are in production, CRM Hub and Desktop Ribbon, and another, the Mobile App, which is being piloted by GAPN to deliver on their commitment to innovate care delivery.
GAPN is using the CRM Hub for care navigation with a focus on mitigating the non-clinical, socio-economic and lifestyle barriers to treatment success faced by some of the highest risk patients their physicians treat.
Care Navigator’s™ Desktop Ribbon is geared to ACO physicians with the belief that GAPN physicians needed a signature innovation outside of their EMR to engage them in accountable care. The Desktop Ribbon has demonstrated promising results and learnings that are being used to shape GAPN's approach to a broader adoption strategy into the next phase of deployment and technology adoption.
The Care Navigator™ Mobile App is truly disruptive innovation, testing the shared belief of both GAPN and BluePrint that patients and their caregivers are ready for new modes of communication and deeper interaction with their providers and health plans. Directed as a patient- and caregiver-centric approach to care management, the Mobile App pilot will, not only, yield insights into the effectiveness of this new mode of communication, but will help direct further expansion of the Mobile App for more patients and their family caregivers.
The Care Navigator™ Mobile App—available for Apple, Windows, Android smartphones -- offers physicians the same features as the Desktop Ribbon, as well as, one-touch calling and secure instant messaging with the entire care team. The Mobile App provides patients and caregivers with real-time access to their care plans and care team members, as well as relevant patient questionnaires and educational materials.
GAPN employs the Mobile App and Desktop Ribbon to provide practices with a one-click, HIPAA-compliant communication tool, enabling quick and efficient instant messaging between physicians, office staff and care coordinators, without having to integrate with their electronic medical record. Additionally, GAPN physicians can view attributed patients, refer those patients into GAPN’s care coordination program, and view care coordination tasks.
The Desktop Ribbon also allows physicians to view the GAPN Quality Performance Dashboard, presenting performance metrics aggregated from Genesis physician practices, which illustrates progress toward goals. Genesis collects and reports these aggregate performance measures so that physicians may view any care gaps and improve upon them. Additionally, the Desktop Ribbon provides a direct link to the GAPN website, where physicians are able to access robust data on their performance.
