El Camino Healthcare District Awards $45,000 Community Grant to Pathways Home Health & Hospice
Sunnyvale, California (PRWEB) January 16, 2015 -- The El Camino Healthcare District (ECHD) awarded a grant for $45,000 to Pathways Home Health & Hospice as part of its community benefit program. The community benefit program was formed to fund organizations such as Pathways Home Health & Hospice that address unmet health care needs of residents in the El Camino Healthcare District. The district includes the communities of Mountain View, Los Altos, Los Altos Hills, Sunnyvale, and a portion of Cupertino.
The Community Benefit Program invests in programs that provide vulnerable populations access to critical health services that may not otherwise be available. As a vital part of the healthcare safety net in the district, Pathways Home Health & Hospice will use these funds to extend home health care to low-income or un/underinsured patients who are recovering from illness or surgery, managing a chronic disease or coping with a life-threatening condition.

This grant will improve the overall health outcomes of these patients and help them stay in their homes and avoid emergency re-hospitalizations.”

About the El Camino Healthcare District
The El Camino Hospital District was established by voter approval in 1956 in accordance with California Local Hospital District Law. The purpose of the District is to establish, maintain and operate or provide assistance in the operation of health facilities and other health care services, provider groups and organizations that are necessary for the maintenance of good physical and mental health in the communities served by the District.
