FamilyWize & the American Heart Association have Joined Forces to Prevent Heart Disease and Stroke
Bethlehem, PA (PRWEB) February 23, 2015 -- FamilyWize Community Service Partnership, dedicated to building strong communities through better health, is proud to join forces with the American Heart Association. As part of a three year commitment, the organizations will raise awareness of cardiovascular disease and the risks associated with it, while improving overall health of people across the country.

Through the collaboration, the organizations will capitalize on their unique strengths to reach individuals and communities with information and tools like FamilyWize’s free prescription savings card and the American Heart Association’s: Life’s Simple 7, which identifies seven health factors and lifestyle behaviors to help people to move toward ideal cardiovascular health; My Life Check, a short survey that provides a personal heart health score and information to help participants to improve problem areas, and EmPowered To Serve, a national online strategic initiative that helps community groups come together to share best practices, resources and results as they work to build a culture of health in their communities.
