Fit4D Launches with Acacia Health Network
New York, New York (PRWEB) January 25, 2016 -- Fit4D announced today that it launched its innovative diabetes platform to members of Acacia Network’s Integrated Service Delivery System. Acacia Network is the second largest Latino non-profit in the country and its network of integrated services provides care for a largely underserved Latino population in NYC, Albany and Buffalo, NY.
Fit4D optimizes the mix between technology and diabetes-expert clinicians, also known as Certified Diabetes Educators (CDEs), to improve health measures in an affordable manner. The platform works with intelligent-scripting algorithms that enable the CDEs to increase capacity 5x to deliver personalized care to members with poorly controlled diabetes.
Most of Acacia Network’s population lives in the Bronx, which ranks lowest or near the bottom for all health and social determinant indicators, mortality and morbidity. As part of their effort to drive transformative change and eliminate health disparities, Acacia is offering Fit4D personalized health coaching to people with diabetes. Fit4D customized their program for this population, its expert diabetes educators engage with Acacia members in both English and Spanish, and through their preferred communication method.

Effective management involves multiple daily medications and often injections, monitoring blood sugar multiple times a day, integrating a healthy lifestyle (nutrition, physical activity, stress management, etc.), and coping with the ongoing psychosocial stresses of the disease. Achieving positive health outcomes is made more challenging as every patient starts in a different place, continues on their own unique journey, responds differently to messaging, and has distinct and varied barriers to management. The challenge of providing effective and on-going support is compounded for most healthcare professionals who have limited time during appointments.
Unlike apps or call centers, Fit4D offers a high-touch, personalized experience for members that leverages proprietary technology and methodologies to demonstrably improve health outcomes and reduces the costs associated with health complications resulting from diabetes (e.g. blindness, neuropathy, kidney disease, etc.).
