(PRWEB) November 24, 2017 -- The Center for Hope Hospice & Palliative Care is known for its two unique residential facilities, Peggy’s House in Scotch Plains and Father Hudson House in Elizabeth, N.J. But many area residents do not realize that they or their loved ones can receive carefully coordinated hospice care in their own homes through the Center.
With November being National Hospice and Palliative Care Month, the Center for Hope wants to remind caregivers, healthcare professionals and individuals at the end of life that its services can be provided wherever a patient resides. This includes private homes or other care facilities, such as nursing homes or other assisted living residences throughout central New Jersey.
“People should be assured that no matter where they are, they can receive hospice services and that the Center remains, in all situations, sensitive to the wishes of our patients and families. If a patient wishes to remain at home, the Center will go out of its way to provide the hospice services necessary to fulfil that desire,” said Frank Brady, president of the Center for Hope.
The Center for Hope serves the terminally ill in Union County as well as parts of Essex, Hudson, Middlesex, Morris and Somerset counties. It offers a range of hospice care services based on an interdisciplinary care plan that is designed by medical directors, nurses, social workers and chaplains. Because the care provided in the community is managed through the Center, it mirrors the experience offered at Peggy’s House and Father Hudson House. Patients have access to a range of supportive and care services focused on meeting their needs and keeping them comfortable, including:
• A Patient Response System that ensures access to an on-duty, awake registered nurse who is ready to visit the home seven days a week, 24 hours a day, no matter what the issue is. This unlimited access, which is uncommon in community hospice care, provides medical attention and palliative care as efficiently as possible.
• All medications, supplies and durable medical equipment directly related to the hospice diagnosis.
• In-home nursing care and/or home health aide visits, depending on patient need.
• Chaplains and social workers for spiritual and emotional support.
• Trained hospice volunteers, who provide companionship and caregiver support.
• Seamless access to the Center’s residences for respite care stays, pain and/or medication management, and caregiver relief. Longer stays, or permanent stays, can also be arranged as your needs change.
