General Medicine, The Post-Hospitalist Company Achieves Readmission Rates Better than 95% of CMS National Peer Group
(PRWEB) February 17, 2015 -- Partner facilities of General Medicine, The Post-Hospitalist Company, attained an average hospital readmission rate of 4.8% for all of 2013, according to the Centers for Medicare and Medicaid Services (CMS). The 2013 Quality and Resource Use Report—compiled by CMS and finalized September 2014—found that General Medicine, The Post-Hospitalist Company, achieved readmission rates 70% lower than all providers within its national peer group. The following quality metrics were considered in these measurements: falls plan of care, advance care plan, high blood pressure screening and follow up, influenza immunization, diabetic foot exam, colorectal cancer screening, and CAD antiplatelet therapy.
