Geneva Woods Pharmacy Brings Transitional Pharmacy Services to the PNW
Vancouver, Washington (PRWEB) July 09, 2015 -- Geneva Woods Pharmacy, Inc. (GWP) (http://www.genevawoods.com), a leading provider of integrated Long Term Care (LTC) and home pharmacy services in under-served rural markets, has announced the introduction of its transitional home pharmacy services in Washington and Oregon. These services are aimed at transitioning patients from acute and post-acute settings to the home setting, as well as providing care for patients with mental health issues. The services will be provided out of GWP’s Vancouver, WA long-term care pharmacy facility.
GWP leverages years of experience in working directly with discharge planners in the Alaska market to bridge the gap that currently leads to significant numbers of readmissions and additional costs to the healthcare system. It is estimated that 60% of all medication errors occur during time of transition of care from one setting to another setting. Furthermore, it is estimated that 72% of post-discharge adverse events for the elderly population are medication-related.
Kim Schneiderman, GWP’s newly-appointed Director of Transitional Pharmacy Services and a long-time expert at designing and implementing specialized pharmacy services, will lead this effort for GWP in Washington. Kim currently is Board President for NAMI Oregon and is actively involved with many other agencies and advocacy groups with the mission of providing care in residential settings.
