HealthMEDX Implements INTERACT™ to Reduce Re-Hospitalizations in Long-term Post-Acute Care
Ozark, MO (PRWEB) October 30, 2013 -- HealthMEDX, the leading LTPAC EMR, announced today at the LeadingAge 2013 Annual Meeting and Expo that HealthMEDX has licensed from Florida Atlantic University the right to incorporate the INTERACT™ Tools into the HealthMEDX Vision solution. INTERACT™ is a quality improvement system that is designed to improve the early identification, assessment, documentation, and communication about changes in the health status of residents in skilled nursing facilities. By incorporating INTERACT™ into the HealthMEDX Vision solution, HealthMEDX potentially reduces the frequency of potentially avoidable transfers to the acute hospital setting, which could result in numerous complications for the patient and waste billions of dollars due to unnecessary health care expenditures.
INTERACT™ provides a set of clinical tools, educational tools and care paths for several of the most common medical illnesses responsible for hospital admissions in elderly patients, including conditions like dehydration, fever, changes in mental status, congestive heart failure, lower respiratory infections and urinary tract infections.
INTERACT™ is a recognized strategy to address the chronic problem of unnecessary readmissions in LTPAC patients. By embedding the INTERACT™ tools directly into the HealthMEDX platform, HealthMEDX customers can focus on improving clinical outcomes while controlling costs.
