Institute for Healthcare Improvement to Feature BluePrint Healthcare IT Progress with Safety Nets using Health IT for Care Coordination in Kansas City Region
Cranbury, NJ (PRWEB) February 26, 2015 -- The Institute for Healthcare Improvement (IHI), well known for its leadership in pursuit of The Triple Aim, has accepted a storyboard about the implementation of health IT at safety net healthcare organizations to optimize care coordination. BluePrint Healthcare IT announced the storyboard will be featured at the 16th Annual International Summit on Improving Patient Care in the Office Practice and the Community in Dallas, TX on March 15-17, 2015. The IHI Summit “will bring together more than 1,000 thought leaders from around the globe, in an intimate and dynamic conference setting, to examine new ways to improve care delivery and coordination across the continuum,” according to the IHI website. The Triple Aim focuses on "better care for individuals, better health for populations and lower per capita costs."
The story, “Safety Nets Implement Care Coordination with Health IT using Optimization Framework and 360⁰ Readiness Assessment,” describes a collaboration of two groups of organizations, known as cohorts, including sixteen healthcare safety net and community mental health organizations in the Kansas City region, led by the Mid-American Regional Council with foundation partners.The safety nets include a mix of providers that deliver primary care and care in mental health, public health, substance abuse assessment, health education and health service coordination. As Project Manager, BluePrint Healthcare IT guided the consortium in developing a region-wide care coordination model using health IT.
BluePrint Principal Consultant, Rebecca Goldberg explained, “We pursue the Triple Aim using BluePrint's three-step Care Optimization Framework – Design-Implement-Optimize – starting with our 360⁰ Readiness Assessment. The assessment reviews and measures organizational culture and values, technology, resources, programs and services to identify immediate and future programs. Working with the two cohorts, we implemented and optimized stages based on commonalities across participants with jointly defined program goals.”
Unmet community needs, including coordinating patient care between safety net organizations, were identified through the 360⁰ Readiness Assessment. This analysis enabled design, architecture and implementation of use cases and additional program opportunities. New programs will build on the foundation and advance the care coordination work of safety net and community mental health organization participants.
