Jvion Named Awardee in Trinity Health Innovation Challenge
Atlanta, GA (PRWEB) December 13, 2016 -- Trinity Health, one of the nation's largest health systems, selected clinical cognitive science leader Jvion as a grant awardee in its inaugural Innovation Challenge. With the award, Jvion will apply the industry’s first and only Cognitive Clinical Science machine to improve care and reduce readmissions for patients who are dually eligible for Medicare and Medicaid. Using a powerful approach to cognitive computing, Jvion will deliver accurate predictions to stop complications and drive effective interventions for one of the most complex and high-risk populations served by the provider.
The dual eligible patient population includes between 9-11 million people within the United States. This group comprises low-income seniors and younger people with disabilities. Individuals covered as dual eligible patients tend to have substantial health requirements, are more likely to have mental health needs, and are more likely to live in a nursing home.

The substantial needs and specific challenges of the dual eligible patient population increase the costs and complexity of delivering care. On average, the annual cost for this patient group is more than double that of non-dual eligible patients. Some of this cost is attributed to higher rates of 30 and 90-day readmissions, which are driven by fragmented care, poor access, disjointed communication across caregivers, and a lack of patient understanding within the dual eligible population.
Jvion will work directly with St. Joseph Health, a Trinity Health ministry in Syracuse, New York, to test and launch the dual eligible use case. The solution will help Trinity Health predict the patients at highest risk of readmission and recommend the best interventions that will reduce risk and drive patient engagement. In addition, Jvion’s solution will help patients, family members, and care providers better coordinate care and communications across multiple settings and organizations by simplifying current processes while eliminating the fragmentation of care.
