According to another article by the Houston Business Journal (“Report shifts blame in fatal Black Elk explosion,” August 21), Black Elk filed a report that claimed that last year’s fatal accident occurred after subcontract workers failed to inform company officials of construction work being done on the platform.
According to the story, subcontract workers were welding in an unsafe area in which they were unauthorized to do so and in direct violation of safety standards aboard the vessel at the time of the accident.
