Magee Rehabilitation Hospital Partners with Locus Health for Launch of Enhanced Lifetime Remote Care Management Program
PHILADELPHIA (PRWEB) October 17, 2017 -- Magee Rehabilitation Hospital is pleased to announce that it has partnered with Charlottesville, VA-based Locus Health, a provider of remote care solutions, to design and implement an innovative program that will enhance the ability of Magee clinical staff to remain connected with patients for their lifetime of follow-up care after an inpatient stay, helping to deliver on the promise of constantly improving patient care. Initially tailored to Magee’s patients with spinal cord injury, the program will allow Magee care coordinators to more easily identify patients at risk of a preventable event like a readmission, and to more easily engage with patients and caregivers on a regular basis, ensuring that patients get the right care and are connected to the appropriate resources at the right time during their recovery.
Magee, recently named a “Best Hospital in Rehabilitation” for 2017-18 by U.S. News & World Report, expects to expand the program to other key patient populations, including those admitted with neurological and stroke-related conditions.

Working closely with Magee, Locus Health is developing a comprehensive navigation platform that will serve as the standard for the remote care of acute rehabilitation patients. Magee care coordinators will utilize the platform to reach out to patients on a timely basis based on predefined post-acute protocols, identifying any issues that need to be addressed, and optimizing the patients’ chances for a smooth recovery in the post-acute setting. The platform will also give Magee actionable data in real-time to achieve the goal of providing consistent lifelong follow-up care to patients and improving internal processes.
