Maine Primary Care Association Implements i2i Systems Platform to Provide Data Warehousing and Population Health Management
San Francisco, CA (PRWEB) June 23, 2014 -- Maine Primary Care Association is now using new insight from i2i Systems’ population health management solution to proactively improve the quality of care its members provide to Maine’s most needy patients.
Seven of Maine PCAs health center members have implemented i2iTracks, the leading population health management program used by more than 1000 healthcare facilities nationwide. Additionally, three of the health centers have now completed implementation of the full i2i Systems solution—i2iTracks at the operational level and PopIQ at the strategic level. By year’s end, MPCA will have 11 health centers implementing i2i Systems solution covering 80 percent of the State’s health center patient population.

“We had the desire to identify and track groups of patients in order to better manage the health of these populations, but we lacked the tool to help get us there,” said Chris Muffett, director of HIT projects for Maine Primary Care Association. “We even tried—and failed—to develop our own data warehouse. It became clear that we needed a tool that health centers could grow locally and trust. That’s when we turned to i2i Systems.”
According to Muffett, the implementation process has served as an early data validation tool. The CHCs have had the opportunity to verify and clean up their EHR data so that it is truly representative of their patient populations and is much more meaningful.
For example, prior to implementing i2iTracks, morning huddle reports existed on a limited basis, if at all. Now, they are used across all seven of the CHCs to proactively prepare for the day’s patients and to identify necessary screenings, labs or follow-up care that can be acted upon while the patient is visiting the health center.
“Since implementing i2iTracks, data no longer exists in a vacuum,” said Muffett. “It is causing the CHCs to examine their workflows, where they are capturing data, and opens the door for broader conversations about how they are providing care.”
Generating required reports for Patient Centered Medical Home, Meaningful Use 2014, UDS and Healthy People 2020 is now a much easier task. In addition, several clinics have implemented practical local searches to help them better understand their patients’ needs, according to Muffett.
PopIQ, is a cloud-based system that aggregates de-identified patient data across multiple organizations and allows for comparative analysis. This is helping Maine PCA identify best practices from its health centers and establish a mentorship program that pairs higher performing centers - in terms of a specific measure or condition - with centers that are not performing as well in this particular area. It also allows users to generate reports and dashboards across a virtually unlimited number of measures and indicators.
“We are extremely excited about the value of PopIQ,”said Muffett. “It is providing us with the bigger picture, a strategic view across all of the participating centers – and it allows health center staff to see how they are performing in the context of their peers across the State.”
