MultiCare Connected Care Selects Sandlot Solutions to Build Clinically Integrated Network
DALLAS and TACOMA, Wash. (PRWEB) April 01, 2015 -- Sandlot Solutions, a leading community health interoperability and analytics provider, announced its partnership with MultiCare Connected Care, LLC, to support clinical interoperability, care coordination, population health management and analytics and reporting for the accountable care organization (ACO).
Sandlot will build a community-wide electronic information exchange platform, allowing both MultiCare-employed and select independent community providers to exchange data, enhancing both care coordination and clinical integration.
MultiCare Connected Care’s Clinically Integrated Network (CIN) of physicians and other care team providers will serve populations throughout the state of Washington, part of a comprehensive approach to healthcare reform focused on delivering patients superior care while lowering overall cost of that care.
