National Healthcare Decisions Day
Sunnyvale, CA (PRWEB) March 13, 2015 -- Sunnyvale based non-profit Pathways Home Health & Hospice will be highlighting the importance of advance directive healthcare planning with an educational event at the Sunnyvale Senior Center on Wednesday April 15. This is part of a coordinated wave of events across the country for National Healthcare Decisions Day. Pathways is providing free information and tools for the community to talk about their wishes with family, friends and healthcare providers--and to complete a written advance directive in accordance with California state laws. (This means naming a healthcare power of attorney and completing a living will.)
The presentation is from 1 - 2 pm in the Laurel Room of the senior center at 550 East Remington Drive. Reserve a seat at 408-730-7361. For more information, visit Resources at PathwaysHealth.org or nationalhealthcaredecisionsday.org.
“As a result of National Healthcare Decisions Day, many more people in our community will have thoughtful conversations with family members about their healthcare wishes and will complete advance directives to make their wishes known,” said Pathways CEO Barbara Burgess. “Fewer families and healthcare providers will have to struggle with difficult healthcare decisions because they don’t have guidance from the patient. And with an advance directive doctors, nurses, and hospitals will be better prepared to honor patient wishes.”
Pathways Home Health & Hospice is a non-profit, community based organization that brings healthcare to people at home. Pathways was founded in 1977 to fulfill the need for professional care at home. That first year Pathways served 12 patients; Pathways now serves more than 5,000 families a year in five Bay Area counties.
Affiliated with El Camino and Sequoia Hospitals, Pathways is accredited by the Joint Commission and is a member of the Visiting Nurse Association of America, California Hospice and Palliative Care Association, and the California Association of Health Services at Home. Pathways has been designated as a 2014 HomeCare Elite Agency for Quality Care.
