Nexus Health Resources Champions Health Management Best Practices at National Readmissions Summit
White Plains, New York (PRWEB) June 22, 2015 -- Nexus Health Resources President and CEO Dr. Virginia Feldman was one of a select number of distinguished thought leaders to share their expertise at the National Readmissions Summit New York 2015. The conference was held June 11 at The Crowne Plaza in White Plains.
The day-long event united local and national industry leaders to share population health management best practices and case studies for hospitals, skilled nursing facilities, and other post-acute providers to help reduce and prevent readmissions. The summit also examined and identified new and existing tools, resources, and services that could assist certain facilities and further prevent readmissions.
“As a practicing physician and executive continually working to help health care providers reduce readmissions, I was honored to speak on today’s medical environment and share my experiences with other heath management professionals,” Dr. Feldman said.
Dr. Feldman and co-presenter Patricia Metzger, director of Case Management Services at Orange Regional Medical Center, touched on Nexus Health Resources’ partnership with the hospital and how to avoid common pitfalls in care coordination during the breakout session. Other topics that were explored at the summit included a panel on improving care transitions with experts like Margaret Deyo, Vice President of Patient Care Services at St. Luke’s Cornwall Hospital.
Dr. Feldman continued, “Our collective goal at this Summit is to find ways to reduce the number of hospital readmissions across the board, and with the powerful brain trust cultivated at this conference, I’m confident we will see results.”
