Nexus Health Resources to Provide Transitional Care Management Services
Middletown, NY (PRWEB) March 03, 2014 -- Dr. Virginia Feldman, MD, Founder, President and CEO, announces the launch of Nexus Health Resources, LLC, and the new company’s signing of an agreement to provide transitional care management and care coordination services to Orange Regional Medical Center (ORMC). Also offering practice management and educational services, Nexus Health Resources is contracted with ORMC to work the hospital’s discharged Medicare patients who have diagnoses of congestive heart failure and pneumonia.
Nexus Health Resources was created to provide hospitals, medical practices and skilled nursing facilities with the administrative support they need to reduce 30-day hospital readmission rates and the resulting financial penalties imposed by Affordable Care Act provisions. “All too frequently, the discharge facility, medical practitioners and other service providers function in silos,” said Dr. Feldman, a practicing surgeon and governing chairperson for Hudson Valley Ambulatory Surgery. “Nexus Health Resources is a partner service improving coordination and communication across care providers, all of which are critical to a patient’s post-hospital recovery. A Nexus Health Resources Care Coordinator engages the patient and their family during hospitalization, collaborates with the hospital’s discharge staff, and then provides the support network the patient needs during the critical 30-day post-hospitalization period. The Care Coordinator performs vital tasks such as scheduling follow-up physician appointments, ensuring delivery of any necessary medical equipment, facilitating delivery of medications to bedside, and providing a recording of care instructions patients can access after discharge.
