ONE Pediatrics Is First in Louisville Area to Achieve Level 3 Patient-Centered Medical Home Designation
Louisville, KY (PRWEB) July 21, 2015 -- ONE Pediatrics is the first group of pediatric medical practices in the greater Louisville area to receive Level 3 Patient-Centered Medical Home (PCMH) Designation. The groups had to pass all 10 major elements in order to receive this prestigious designation.
Patient-centered medical homes provide care with an emphasis on the physician-patient relationship. They provide personalized, efficient, and comprehensive care while continuing to emphasize the personal relationship between patients, families, and their physician. Physicians in offices with the patient-centered medical home designation lead care teams that embody these values. This includes meeting the patient’s needs for preventative care, care of chronic medical conditions, 24-hour physician access, providing quick responses to acute illness, and coordination of care with associated subspecialists. One of the main goals of the program is to facilitate the patient’s ability to take a more active role in their care.

Obtaining PCMH certification was physician driven and required input and work from each of us, as well as experienced office personnel. As part of a merged group, we were able to share resources and ancillary services to better meet the requirements. Being recognized as a PCMH is evidence that we will continue to provide our patients with easy access, increased communication and the latest electronic information technology all while meeting performance and quality measures,” said Dr. Casey Lewis of Pediatrics of Bullitt County.
Six of the seven groups that make up ONE Pediatrics have filed for and received the Level 3 PCMH designation. The seventh group is set to file this summer.
